2022 Scholarship Application Form

FACES National Scholarship Program provides equal opportunity to all applicants. There are no restrictions based on race, ethnicity, gender or social class.

Students must meet the following requirements to be eligible:

• Full-time graduating US High School Senior in good academic standing

• Minimum GPA 2.75 out of 4.0 scale or 3.75 out of 5.0 Scale

• Attending an accredited 2-yr or 4-yr college or university in Fall 2022.

• Students must be a citizen or permanent resident of the United States.
